PARENTS of TRIP STUDENTS: If your child is going on the trip to Philly/NYC and they take any type of medication
on a regular basis, that medication MUST be turned in on Tuesday morning, November 25th to the chaperone in the band room.
A chaperone will be collecting the medications as well as an "Overnight Trip Medication Form" from 7:00AM until the students
leave on the buses. The medication must be in a container that has the original prescription on it (such as the medicine
bottle, box, inhaler, etc.) which shows your child's name, the name of the medication, the dosage and the instructions for
administration. If your child has multiple medications please put them in a Ziploc bag with the child's name written on the
bag. A completed form MUST be turned in with your child’s medication and your signature MUST be on the form.

be turned in Tuesday morning. While forms will be available on Tuesday morning, the 25th, in the band room; it will save a lot
of time if you already have the form completed. If the form is incomplete and/or not signed, you will have to come to the
school to complete the form before the band leaves on the buses.
